Cardiovascular disease (CVD) remains a leading cause of morbidity and mortality worldwide, creating substantial pressure on healthcare systems and communities. Although evidence supports structured cardiac rehabilitation (CR), medication adherence, and preventive cardiovascular strategies, major implementation gaps remain, particularly in community settings where access and continuity of care are limited. The increasing use of wearable technology and remote patient monitoring (RPM) platforms presents an opportunity to address these challenges. This review examines current evidence regarding the integration of wearable devices and RPM into cardiovascular rehabilitation, medication adherence support, and preventive cardiovascular care in community environments. It discusses the technological
landscape, clinical applications, barriers to implementation, equity concerns, and emerging innovations such as artificial intelligence-driven analytics and integrated digital care models. Current evidence indicates that wearable-enabled RPM may improve exercise participation, physiological monitoring, medication adherence, and early identification of cardiovascular deterioration outside traditional healthcare settings. Wider adoption of these technologies, supported by effective clinical workflows and equitable access strategies, may contribute to improved cardiovascular outcomes and reduced disease burden.
